PRACTICAL DETAILS FOR YOUR NEXT STEP

A practical guide for your family.

Helpful details, clear questions and the information to prepare before making an arrangement.

01

Before discharge

Collect the written plan, current medication list, follow-up appointments and advice on who to contact if there are concerns. Ask the ward about the discharge timetable and any support that must be organised first. Confirm who will explain the instructions and where questions about treatment should be directed after returning home.

02

Before the journey

Confirm transport, mobility needs, access to the home and any equipment the hospital recommends. Check the destination address, home access and planned arrival time. Share any change in the discharge date promptly so the caregiver’s proposed start can be reviewed.

03

Before the first shift

Agree the care schedule, key contacts, daily tasks and clinical responsibilities. Show where essential supplies and instructions are kept. Arrange a family contact for the first handover and explain where the agreed supplies are kept. Show the caregiver the written responsibilities and distinguish them from clinical instructions for a nurse.

04

After arriving home

Follow the discharge plan and raise concerns with the appropriate treating team. Request a review if the level of support is no longer suitable. Keep questions about recovery for the treating team and questions about booked hours for MCS. Review whether the first week’s schedule still fits the family’s actual support needs.

05

Questions to settle before the first shift

Ask who will receive the caregiver, where instructions are kept and how the family will communicate updates. Confirm the shift hours against the time the patient is expected home. If discharge is postponed, notify MCS so the proposed start date and staffing can be reviewed.

  • Identify the handover contact
  • Confirm the expected arrival time
  • Notify discharge delays
06

Reviewing the first week at home

Use the first week to check whether the booked hours fit the household routine and whether relatives know their responsibilities. Keep follow-up questions for the treating team and booking questions for MCS. When the support required changes, request a review of the agreed schedule and duties.

  • Review hours and responsibilities
  • Keep follow-up questions organised
  • Discuss any revised care scope
